It is important to know whether your insurance is accepted before scheduling an appointment to ensure that your visit will be covered. At our office, we strive to make healthcare accessible and affordable for everyone. We accept a range of insurance plans to provide our patients with quality care tailored to their needs.
We encourage you to reach out to our office to confirm that your insurance plan is accepted by your clinician so that we can assist you with scheduling your appointment, submitting claims, and answering any questions you may have about your insurance benefits.
Please understand that not every clinician accepts every insurance at PLC. Also, while we are currently in the process of becoming a Medicare provider, we are not accepting Medicare patients at this time.

INSURANCE AND PROFESSIONAL Fees
Insurance benefits have become increasingly more complex. Our agency will assist in ascertaining information regarding your coverage, but you are responsible for knowing your coverage and for letting us know if/when your coverage changes. You will be responsible for any fees not covered by the insurance (including claims that process differently than our original estimation). These fees—which include co-pays, co-insurances, or deductibles—will be collected at the time of your appointment. If you fail to notify the office of any insurance changes you may be responsible for any noncovered charges.
You should also be aware that most insurance companies require you to authorize our agency to provide them with a clinical diagnosis as well as additional clinical information.
Our out-of-pocket professional fees are as follows:

In addition to these set services, it is our practice to charge this amount on a prorated basis for other professional services that may be required such as telephone conversations that last longer than 15 minutes, attendance at meetings or consultations which have been requested, or the time required to perform any other service requested of us. If you anticipate becoming involved in a court case, we recommend that we discuss this fully before you waive your right to confidentiality. If your case requires our participation, you will be expected to pay for the professional time required even if another party compels us to testify.
You are responsible for any financial obligations related to your treatment at the time of your appointment unless prior arrangements have been made. Any checks returned to our office are subject to an additional fee of up to $35.00 to cover bank fees. If you refuse to pay your debt, you may be discharged from the practice, and PLC reserves the right to use an attorney or collection agency to secure payment.
FINANCIAL AGREEMENT
All consumers seen through PLC are responsible for payment of services received. PLC will attempt to collect payment from active insurance policies based on the information provided by the consumer; however, payment is ultimately the consumer’s responsibility. Any fees that we are unable to collect from the insurance company for any reason (i.e. co-pays, deductibles, lapse in insurance, etc…) will be paid by the consumer or their legal guardian(s). If payment does not occur within 3 months of the appointment, PLC has the right to discharge the patient from care in addition to sending the consumer’s account to a collection agency. Once an account has been sent to collections, PLC will not accept the consumer back for future services.
It is the consumer’s responsibility to inform PLC staff of any changes in insurance status. If this information is not conveyed, the consumer will be responsible for all fees incurred.
Procedure: The following steps will be taken to obtain patient payment:
- All patient insurance information is collected and verified prior to an appointment being made.
- If a patient requests a self-pay rate, they are informed of the rates prior to being scheduled.
- A financial responsibility form is signed by the consumer or their legal guardian(s) informing of financial expectations and responsibilities.
- All co-pays or self-pay fees are due the day of the appointment; if the consumer does not have payment that day, their appointment may be rescheduled at the discretion of PLC.
- All deductibles, additional out-of-pocket expenses, or fees owed due to an issue with insurance, will be sent via patient statement for collection.
- Communication regarding financial responsibilities on both sides is imperative. If 3 statements have been sent, and no communication has occurred with the billing department, the consumer may be discharged from services and the account may be sent to collections.
- If a consumer is discharged due to non-compliance with payment, they will no longer be eligible for future services with PLC.
